Vaccine Schedule 

The table above is the recommended schedule for immunizations at each age. The following list is how our office typically administers each immunization at each well child visit according to age. Some vaccines are given in combination together in one shot

1 week    No vaccines

1 mo       No vaccines

2 mo       DTaP #1, Hib #1, IPV #1, Prevnar #1, 

                Hep B #1 (3 shots)

4 mo       DTaP #2, Hib #2, IPV #2, Prevnar #2,  

                Hep  B #2 (3 shots)

6 mo       DTaP #3, Hib #3, Prevnar #3, IPV #3,

                Hep B #3, Flu (3-4 shots)

9mo        No vaccines

12mo      MMR #1, Varivax (2 shots)

15mo      DTaP #4, Hib #4, Prevnar #4 (2 shots)

4-6 yrs    DTaP #5, IPV #4, MMR #2

14-16 yrs     Td (tetanus booster)

*Catch up immunizations for MMR #2 and Hep B series for those who have not yet been immunized usually ranges in ages from 7-18yrs.
